The Society is committed to meeting or exceeding the privacy standards established by federal regulators and industry bodies and will comply with federal laws, including the Personal Information Protection and Electronic Documents (PIPED) Act.

We are dedicated to protecting your privacy and safeguarding your personal, business, and financial information. It is one of our highest priorities and remains the cornerstone of our commitment to you. However you choose to provide information to us, we use vigorous security safeguards and adhere to ten privacy principles to ensure your personal and financial information is protected.

Privacy Principles

In order to insure that your information is protected and to comply with privacy legislation, Horizon Achievement Centre has adopted the following principles:

Principle 1 – Accountability
The Executive Director is accountable to the Board of Directors for Compliance with this policy. Each HAC Staff and Board member is responsible for maintaining and protecting the collected information under their control. They are responsible for the management of the policy including compliance with the Ten Privacy Principles.

Principle 2 – Identifying Purposes
Horizon Achievement Centre only collects personal information from client/participants, prospective clients/participants, staff, and board members necessary to provide attentive and responsible programs and to ensure we can provide emergency health care, ensure a safe and secure environment, fulfill Funders’ information requirements and meet applicable regulations. The purposes for which information is collected shall be identified before or at the time the information is collected. Information is kept only as long as required to fulfill the purposes identified, unless permission is obtained from the individual providing the information.

Principle 3 – Consent
The knowledge and written consent of the information provider is required for the collection, use or disclosure of personal information except where required or permitted by law.

Principle 4 – Limiting Collection
All information collected shall be limited to those details necessary for the purposes identified by Horizon Achievement Centre. Information will be collected by fair and lawful means.

Principle 5 – Limiting Use, Disclosure and Retention
Information will only be used or disclosed for the purpose for which it was collected unless the provider has otherwise consented, or when it is required or permitted by law. Information will only be retained for the period of time necessary to comply with Funders’ requirements and government regulations.

Principle 6 – Accuracy
Provided information will be maintained in as accurate, complete and up-to-date form as is necessary to fulfill the purposes for which it is to be used. Information providers have the right to view stored information to ensure that it is accurate. Requests should be made in writing to the Executive Director.

Principle 7 – Safeguarding Customer Information
Information is protected by security safeguards that are appropriate to the sensitivity level of the information. Filing cabinets are locked and in a secure area. Electronic files are password protected. Employees are bound by confidentiality agreements.

Principle 8 – Openness
Horizon Achievement Centre will make information available to providers concerning the policies and practices that apply to the management of their information.

Principle 9 – File Access
Upon written request, an information provider shall be informed of the existence, use and disclosure of their information, and shall be given access to it. Information providers may verify the accuracy and completeness of their information, and may request that it be amended, if appropriate.

Principle 10 – Handling Complaints and Suggestions
Information providers may direct any questions or enquires with respect to the privacy principles outlined above to the Executive Director of the Horizon Achievement Centre. To appeal a decision made by the Executive Director, written details should be directed to the Chairperson of the Board.

Personal Information Defined

The PIPED Act defines personal information as any factual or subjective information, recorded or not, about an identifiable individual.

It includes but is not limited to information in any form such as: age, name, ID numbers, income, ethnic origin, opinions, evaluations, comments, disciplinary actions, credit records, medical records, disputes, intentions, etc.

It does not include the name, title, business address or telephone number of any employee of an organization.
